Tirzepatide Works On Two Receptors, Not One
Tirzepatide is the active ingredient in Mounjaro and Zepbound. Unlike semaglutide, which acts on the GLP-1 receptor, tirzepatide acts on both GLP-1 and GIP. GIP is a second incretin hormone involved in insulin response and fat metabolism, and adding it changes how the medication behaves. In the SURMOUNT-5 trial, the first direct head-to-head comparison of the two medications, tirzepatide produced greater weight reduction than semaglutide over 72 weeks, along with greater improvements in blood pressure, HbA1c, fasting insulin, triglycerides, and HDL cholesterol. Body composition data also suggests tirzepatide loses a smaller proportion of lean mass than semaglutide, though both require the same protein and resistance training approach.
